The Transitions team is committed to recognizing that individuals and families are the true experts in their own care and decision makers in the development of an individualized plan of care. We focus on helping keep our patients and their families free from pain, anxiety, and stress. Our hospice team includes Medical Directors, Registered Nurses, Social Workers, Chaplains, Certifed Nursing Aides, and Volunteers dedicated to providing individuals what they need to ensure a quality of life during a difficult time.

WHAT IS HOSPICE?


Hospice care is available and appropriate for anyone who has made the decision, along with their families and doctors, to amend their goals from curative to comfort. Assuming the disease process follows its normal course, any person certified by their attending physician and to have a life expectancy of six months or less is a candidate for hospice care. Hospice provides services that promote the physical, emotional and spiritual needs of the patient and family in the patient’s home, or wherever the patient resides.
